Between sessions, patients carry how they sleep, how much energy they have, how symptoms develop. The practice typically only sees the snapshot on appointment day. With the Centi · Elara integration that turns into a trajectory — controlled, aggregated, revocable at any time.
The appointment is over, the patient goes home. What happens over the next weeks — how sleep reacts, whether exhaustion tips over, how PEM episodes distribute — falls between the chairs. At the next appointment it gets reconstructed. Rarely precisely, often with gaps.
The Elara app is the tracker patients use to close that gap. Centi-Health shows the trajectories aggregated inside the patient record — without any raw data or diary entries being exposed anywhere.
Inside the patient record the trajectories appear as calm, aggregated panels — no micro-logging, no diary entries, no raw data. The question stays clinical: how does sleep develop over four weeks? When do PEM episodes cluster? What does the patient do between visits?
Sleep duration, wake phases and quality — aggregated over weeks, not nights.
Self-reported energy across the day, smoothed into a trend curve.
Heart rate, HRV and activity from the wearable — as a time series per marker.
Symptom trajectories without diary entries — frequency, intensity, clusters over time.
Post-exertional malaise episodes: distribution, trigger heuristics, recovery duration.
Each area is a separate decision by the patient — what is not released will not appear inside the practice.
The connection forms exclusively when the patient actively enters the code from the practice. After that, inside the Elara app, they pick which areas to share. This decision is not final — every area can be withdrawn at any time.
If the connection is revoked later, the aggregated data disappears from the Centi-Health record. No shadow remains — no "last state" archived by the practice.
Not more data — but the right data, at the right time, with explicit consent.
The first step happens in the practice, the other two inside the patient’s Elara app. Nothing flows before.

Generate a 6-digit connection code inside the patient record. Valid for 30 days. Hand off verbally or by email from the practice environment.
The patient installs the Elara app and enters the code during onboarding. Only then is a connection established — no data flows before.
A separate decision per area (sleep, energy, vitals, symptoms, PEM) inside the app. What is not released, the practice does not see.
Centi-Health and Elara store on EU infrastructure, GDPR-compliant.
The code alone connects nothing — only active entry inside the Elara app establishes the link.
The practice sees summarized trajectories. Diary entries, notes, detail triggers stay inside the app.
If an area or the entire connection is revoked, the aggregated data vanishes from the patient record.
